Intro Coaching Call

This intake form MUST be completed at least 24 hours before our session. You will not be able to book a call with me any sooner than 24 hours in advance.

Please be sure to check your email for form completion confirmation. If the form email confirmation is not received automatically, then it was not received on my end and will need to be resubmitted. 

Coaching call appointments booked will be cancelled if form is not received at least 24 hours prior to scheduled session. Your session time is reserved exclusively for you. Because of this, all bookings are non-refundable but may be rescheduled within the guidelines below:

• Sessions may be rescheduled once with at least 24 hours’ notice. Rescheduling requests made within 24 hours of your session will be considered a late cancellation. 

• Sessions canceled within 24 hours are considered completed and are not eligible for rescheduling.

• If you do not attend your scheduled session and have not communicated prior, the session will be marked as completed and will not be rescheduled.
